COVID-19 Tier 1B Vaccine Registration

Please complete the COVID-19 Vaccine Administration Record and bring the completed form with you to your scheduled clinic.

Ohio has developed a central coronavirus vaccine registration website that will allow people to sign up for the vaccine and is working to add providers to the system. A link will be posted here when available.

For more information about the vaccines, please read the Moderna or Pfizer Emergency Use Authorization.

Tier 1A Vaccine Registration

At this time, the Trumbull County Combined Health District (TCCHD) and the Warren City Health District (WCHD) are working together to plan the administration of vaccine to Tier 1A-eligible persons. Vaccine is in very limited supply and is expected to stay in limited supply for the near future. As such, TCCHD and WCHD will be scheduling limited small vaccine clinics based on the number of vaccines we have on hand at the time of the clinic. Clinics will be by invitation only.

Please complete the registration survey at . Your answers will allow us to plan for future vaccine clinics. Based on your answers, our health department staff will contact you with the date and time of the clinic that you should attend. If for any reason your answers exclude you from Tier 1A, we will hold your information for a later tier. Please understand that even if you are Tier 1A-eligible, it may be several weeks until your clinic is scheduled. All scheduling will be based on vaccine availability.

Due to Governor Dewine’s Stay at Home order and practicing physical distancing during this coronavirus spread the Trumbull County Combined Health District is closing our doors to the public beginning Tuesday, March 24, 2020. 

Arrangements will be made to drop off materials at our front door or feel free to mail paperwork to our office at 176 Chestnut Avenue NE, Warren, OH 44483. 

If there is a need for cribs, car seats and naloxone please call ahead at 330-675-2489, option #3 to schedule a pick-up time with our staff members.

Due to the high volume of COVID-19 complaints regarding violations of the Stay at Home Order along with reduced staff, we have setup a phone number that the public can call to leave their COVID-19 complaints. We will look into all complaints as soon as possible. The number is 330-675-7841
